Ownership Change

Listed below are some common situations that result in a change of ownership. Please note the appropriate situation and forward the requested documents to JP Oil Company, LLC. Please also include the new owner’s full name, address, phone number and social security number. Ownership changes will be implemented in the system as soon as possible upon receipt by JP Oil, however, we must receive all of the appropriate documents by the 10th of the month to ensure the ownership change is made before the following check run.

SITUATION PROVIDE THE FOLLOWING DOCUMENTS
Change in Ownership Due To Death of an Owner (died intestate or without a Will) Death certificate and Affidavit of Heirship. Click here for affidavit form and instructions.
Change in Ownership Due To Death of an Owner (died testate or with a Will) Death certificate and copy of Will. If the Will has been probated, also provide probate documents.
Sale of An Interest Copy of deed, assignment or other conveyance instrument as filed of record in the county or parish where the property is located.
Transfer of Ownership To A Trust Copy of Trust Agreement or Memorandum of Trust along with recorded conveyance in appropriate county/parish.
Name Change A photocopy of marriage certificate, divorcee decree or other document affecting the name change.
Change In Ownership Due to Divorce Complete Divorce Decree including any conveyances made and filed of record in the appropriate county/parish.
Company or Corporation’s Name Change Certificate of Name Change
Company or Corporate Merger Certificate of Merger. Include new Tax I.D. Number
Appointment of Power of Attorney or Guardian Copy of Power of Attorney or Letters of Guardianship
Bankruptcy – Chapter 7 Order naming Trustee of Bankruptcy Estate and appropriate recorded conveyance.
